CoreFit Wellness, LLC — Effective September 1, 2026

These terms govern your use of corefit-wellness.com, form.corefit-wellness.com, our online booking and reservation tools, and our text messaging program. By using them, you agree to these terms.

SMS MESSAGING PROGRAM

Program description: CoreFit Wellness sends text messages to customers who opt in. Messaging use cases include reservation and booking confirmations, class and appointment reminders, booking follow-ups, membership updates, and customer support replies. With your separate consent, we may also send occasional updates and offers about CoreFit and FORM.

Opting in: you opt in by checking the consent box on our website form, by consenting during online booking, or by giving verbal consent by phone and confirming by text.

Opting out: text STOP to any message at any time to cancel. You will receive one confirmation text and no further messages. Text START to resubscribe.

Message frequency: varies based on your bookings and activity.

Message and data rates: message and data rates may apply according to your mobile carrier plan.

Carrier liability: mobile carriers are not liable for delayed or undelivered messages.

Age restriction: you must be 18 years of age or older to opt in to and receive text messages from CoreFit Wellness.

Consent to receive text messages is not a condition of purchasing any service or membership.

RESERVATIONS AND BOOKINGS

Reserving a FORM founding spot or booking a class online holds a place for you; it is not a purchase, and no payment is collected on the website. Membership is confirmed in person and payment is arranged at your first visit.

FORM classes are limited to five participants. Booked classes may be cancelled up to 8 hours before the class start time to retain your class credit. Cancellations inside 8 hours, and no-shows, forfeit that reservation.

Founding-rate memberships are limited in number, offered on a first-confirmed basis, and may be withdrawn once the founding allocation is filled.

Schedules, instructors, and offerings may change. We will make reasonable efforts to notify booked participants of changes.

HEALTH AND PARTICIPATION

Exercise carries inherent risk. By participating you confirm you are physically able to do so and will inform your coach of any injury, condition, or limitation. Our staff do not provide medical advice; consult a physician before starting any exercise program. Participation is subject to our studio waiver and policies, provided at your first visit.

WEBSITE USE

Content on our sites is for informational purposes and belongs to CoreFit Wellness, LLC or its licensors. Do not misuse the site, submit false information, or interfere with its operation. We may suspend access for misuse.

DISCLAIMER AND LIMITATION OF LIABILITY

Our websites and messaging program are provided "as is." To the fullest extent permitted by law, CoreFit Wellness, LLC is not liable for indirect, incidental, or consequential damages arising from use of the website or messaging program.

GOVERNING LAW

These terms are governed by the laws of the State of Florida. Any dispute will be handled in the state or federal courts located in Lee County, Florida.
